By filling up any online application form or enquiry forms or forms on our website, you agree that Integrated Community Services may collect, use and/ or disclose your personal data, as provided in this application form for the following purposes:
- Evaluating your application to be a member with us;
- Administering and managing your relationship as our member, including fees, your health condition, your financial condition, social activities and programmes
- Dealing with Members Experience activities. “Members Experience” refers to activities, (e.g. festival celebrations, group ceremonies, orientation and member group interaction, conferences, programmes, members benefit activities) which may be provided by Community Integration to enrich your life as a member with us, create networking opportunities for you, foster community spirit within the community, encourage holistic development of members both inside and outside the home and to assist you in planning and preparing for your life as senior;
- Taking of photographs and/or videos (whether by Community Integration staff or third party photographers and/or videographers) during events or seminars organised by Community Integration or its affiliates for publicity purposes (including in Community Integration’s publicity and communications materials).
- Maintaining your relationship as our member, including informing you of promotions, benefits and services available to member, and organising, facilitating and notifying you of member events and programmes, courses or other activities, and providing you with Community Integration publications that are for member.
